Job summary

ComForCare Home Care in New Jersey is seeking a Registered Nurse to conduct thorough client assessments, develop personalized care plans, and implement interventions to achieve client goals.

You will travel between sites to provide care, administer medications when needed, maintain detailed records, and coordinate referrals with other healthcare professionals.

Qualifications

• Must be a graduate of an accredited school of professional nursing.
• Current RN license in the state of practice (NJ).
• CPR certification is required.
• Demonstrated knowledge of safety and infection control practices.
• Reliable, insured personal vehicle and valid driver’s license required.

Responsibilities

• Perform comprehensive assessments of client status (physical, psychosocial, environmental).
• Develop individualized care plans based on data analysis and current findings.
• Provide interventions to achieve realistic client outcomes.
• Report changes to update the plan of care to reflect progress.
• Administer medications as needed and maintain detailed client records.
• Initiate referrals to other health team members as required.
• Travel between sites for client visits and assessments.

What we are looking for:

Salary Range is based on experience.
