Job summary

Optum Home & Community Care is seeking Nurse Practitioners or Physician Assistants to perform in-home assessments across designated areas in Charleston, SC. Daily travel to patient homes is required; no night shifts, weekends, or holidays.

Qualifications

• Active and unrestricted licensure in the state you will practice.
• ANCC or AANP Family/Adult/Geriatric Primary Care certification required for NPs.
• NCCPA certification and state licensure for PAs.
• DEA licensure or prescriptive authority (where allowed).
• Driver's license with access to reliable transportation.

Responsibilities

• Conduct in-home assessments, including medical history, vitals, and exam.
• Manage patients through acute events or chronic condition flares.
• Coordinate with interdisciplinary care team and document ICD-10 codes.
• Perform advanced illness and end-of-life conversations when appropriate.
• Provide counseling and education to patients and caregivers.

Education
