Job summary

Optum is seeking a Nurse Practitioner or Physician Assistant for in‑home care assignments across the Florence, SC area. You will conduct comprehensive in‑home assessments, review medical history, perform vital signs, order tests as appropriate, and coordinate with an interdisciplinary team to manage acute episodes and chronic conditions.

Daily travel to patient homes with no nights, weekends, or holidays, a $10,000 sign‑on bonus for external candidates, and an emphasis on delivering

Qualifications

• NP: Active ANCC or AANP certification in Family, Adult, Geria‑tric, or Adult-Gerontology Primary Care.
• PA: Current NCCPA certification and state licensure.
• Active, unrestricted license in the practicing state by start date.
• Valid driver’s license; reliable transportation.
• DEA licensure/prescriptive authority or ability to obtain post-hire as allowed by state regulations.

Responsibilities

• Conduct comprehensive in-home assessments, including medical history review, medications, vitals, and physical exam.
• Manage acute medical situations and chronic condition exacerbations.
• Coordinate with interdisciplinary care team and communicate plans of care.
• Document thoroughly, including ICD-10 coding.
• Provide counseling and education to patients and caregivers.
