Job summary

UnitedHealth Group in Greenville seeks a Nurse Practitioner or Physician Assistant for in-home care, with no nights, weekends, or holidays. Daily travel to patient homes within the assigned area is required.

You will join Optum Home & Community Care, working with an interdisciplinary team to assess, manage, and coordinate care for high-need beneficiaries in their homes. This role emphasizes compassionate, preventive care and patient education.

Qualifications

• Active certification as NP or PA (Family, Adult, Geriatric, or Adult-Gerontology Primary Care)
• Current, unrestricted state licensure to practice
• Active, unrestricted driver's license and reliable transportation
• DEA licensure/prescriptive authority or ability to obtain post-hire
• Ability to establish a collaborative practice agreement where required
• Ability to travel daily to patient homes and navigate varied environments

Responsibilities

• Conduct comprehensive in-home assessments including med history review, vitals, and physical exam
• Manage patients through acute medical events or chronic condition exacerbations
• Oversee short-term sub-acute rehabilitation processes
• Ensure accurate documentation including ICD-10 codes
• Collaborate with interdisciplinary care team
• Perform advanced illness and end-of-life discussions
• Provide patients and caregivers with counseling and education
