Job summary

Optum Home & Community Care is seeking a Nurse Practitioner to provide in-home clinical care across assigned patient homes in North Carolina. Daily travel to patient homes is required, with no night, weekend, or holiday shifts.

Responsibilites include comprehensive in-home assessments, managing acute and chronic conditions, and collaborating with an interdisciplinary team to ensure high-quality care. Competitive compensation and benefits offered.

Qualifications

• Active NP certification in Family, Adult, Geriatric, or adult-gerontology primary care.
• Active, unrestricted state license with ability to start by start date.
• Current DEA licensure or ability to obtain post-hire.

Responsibilities

• Conduct in-home assessments with past medical history review, medication reconciliation, vital signs, and physical exam.
• Manage members through acute medical situations or exacerbations of chronic conditions including testing and interventions.
• Manage members during short-term sub-acute rehabilitation episodes.
• Ensure accurate and complete documentation, including ICD-10.
• Communicate and collaborate with the interdisciplinary care team.
• Conduct advanced illness and advanced care planning conversations.
• Provide patients and caregivers with counseling and education.
