UnitedHealth Group is expanding its Optum HouseCalls team, delivering in-home health assessments to Medicare Advantage and other plan members. This field-based role focuses on education, gap closure, and collaboration with PCPs to improve outcomes.

The position supports flexible scheduling, no on-call, and home-based care. Active NP/PA credentials and state licensure are required, with travel to patient homes as needed.

Qualifications

• Active certs in Family/Adult/Geriatric Primary Care required (ANCC or AANP).
• NCCPA certification and state licensure required for PAs.
• Active, unrestricted licensure in practicing state (or ability to obtain).
• Valid driver’s license and access to reliable transportation for home visits.
• Ability to perform in varied home environments, with adaptability for assessments.
• If independent practice is allowed, new hires must pursue board approval.

Responsabilités

• Conduct comprehensive in-home assessments including history, meds, vitals, and exams.
• Document diagnoses for care management and planning.
• Share findings with PCPs to close gaps in care.
• Identify urgent situations and intervene appropriately.
• Educate members on diseases, meds, and adherence.
• Address social determinants of health and provide referrals.
