Job summary

UnitedHealth Group's Optum HouseCalls team seeks an Advanced Practice Clinician (NP or PA) to perform in-home health assessments for Medicare Advantage and other plan members. This field-based role emphasizes education, gap closure, and collaboration with primary care providers.

Qualifications

• Active ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gero Primary Care
• Active NCCPA certification and state licensure
• Active and unrestricted licensure in the practicing state
• Driver's license with reliable transportation for home visits

Responsibilities

• Conduct comprehensive in-home assessments including history, vitals, and exams
• Document diagnoses for care management and treatment planning
• Communicate findings to PCPs and address care gaps
• Identify urgent situations and intervene appropriately
• Educate members about diseases, medications, and adherence
• Address social determinants of health and provide referrals as needed
