Job summary

Optum Home & Community Care is hiring a HouseCalls Advanced Practice Clinician (NP or PA) to conduct in-home health assessments for Medicare Advantage members. This field-based role emphasizes education, gap closure, and collaboration with PCPs, with no prescribing required.

Flexible scheduling is provided to support work-life balance, with coverage across the 7-day week and no on-call, weekends, or holidays.

Qualifications

• Active certifications for NP/PA in primary care
• Experience with home health preferred
• Ability to operate in diverse home environments, safety awareness

Responsibilities

• Conduct in-home assessments for Medicare Advantage members
• Document diagnoses for care management and treatment planning
• Communicate findings to PCPs to address care gaps
• Identify urgent situations and intervene appropriately
• Educate members on diseases, medications, and adherence
• Address social determinants of health and provide referrals as needed
