Job summary

Monogram Health in Iowa City is seeking a Care Manager - Registered Nurse to join a home-based care team. You will build trusting relationships with a small panel of patients, develop and adapt care plans, and support high quality of life at home.

You will perform in-home visits, monitor biometric data, coordinate with physicians and pharmacists, and educate patients and families on CKD, dialysis, and proactive care decisions.

Qualifications

• Graduate of an accredited School of Nursing.
• Currently licensed as a Registered Nurse in the State of the posted location.
• 2+ years experience in care management and/or with CKD/ESRD.
• Ability to travel locally for in-home visits.
• BLS certification required; onboarding support provided by the company.

Responsibilities

• Develop and adapt care plans with patients’ medical providers.
• Perform in-home care management visits to execute care plan.
• Monitor biometric data and follow protocols for interventions.
• Coordinate with pharmacists and prescribers for medication management.
• Educate patients on CKD, ESRD, dialysis and related comorbidities.
• Coordinate with dialysis providers to ensure smooth transitions of care.
