Job summary

Monogram Health in Davenport, IA seeks a Care Manager RN to lead in-home care for patients with chronic kidney disease. You will collaborate with clinicians to create dynamic care plans, visit patients in their homes, monitor health data, and educate patients and families about treatment options.

Qualifications

• Minimum 2+ years in care management and/or CKD/ESRD care.
• Excellent verbal communication with patients and physicians.
• Familiarity with Microsoft Office and mobile/web apps.

Responsibilities

• Develop and adapt patient care plans with providers.
• Perform in-home care management visits and assessments.
• Monitor biometric data and intervene per protocols.
• Reconcile medications and coordinate with pharmacists.
• Educate patients on CKD/ESRD, meds, diet, exercise, and lifestyle.
• Coordinate with dialysis providers for seamless transitions.
