Monogram Health seeks a skilled Registered Nurse to join our Care Manager team. You will work with a small patient panel, perform in-home visits, and collaborate with providers to adapt care plans that improve quality of life for patients with chronic conditions.

The role requires BLS, NJ RN licensure, 2+ years in care management, strong communication, and the ability to travel locally for in-home visits.

Competenze

• Graduate of an accredited School of Nursing.
• Current Registered Nurse license in the posted location (New Jersey).
• Experience in care management and CKD/ESRD is preferred.

Mansioni

• Develop and adapt individualized care plans with patients and their medical providers.
• Perform in-home care management visits to execute care plans.
• Monitor biometric data and intervene per approved protocols.
• Coordinate medications with pharmacists and prescribers.
• Educate patients and families about CKD/ESRD, dialysis, medications, and lifestyle choices.
• Support adherence and proactive care decisions, including Advance Care Plans.
