Job summary

Humana is seeking a Care Coach 2 to assess and plan care for Medicaid/dual-eligible members in Illinois, focusing on home and community settings. You will evaluate needs and coordinate resources to support optimal wellness.

You will travel locally to member homes and facilities (75-90%), use EMR systems, and collaborate with interdisciplinary teams to implement member-specific care plans.

Qualifications

• Post-degree clinical experience in behavioral health.
• Active Illinois license in LCSW, LMFT or LCPC (No supervisees or provisional licenses).
• Experience with Medicaid/dual-eligible populations is preferred.

Responsibilities

• Ensures members are receiving services in the least restrictive setting by assessing care needs.
• Plan and implement interventions to meet care needs.
• Coordinates services, monitors and evaluates the care plan against member goals.
• Guides members/families towards resources appropriate for their care.
• Visit Medicaid members in their homes, Assisted Living Facilities, and/or Long Term Care Facilities; 75-90% local travel.
