Job summary

PACE by IIH is seeking an Entry Level Home Care Aide in Stockton, CA to join an interdisciplinary team delivering personalized care in participant homes. The role supports daily living activities, monitors participant condition, and ensures safety and confidentiality in alignment with center policies.

The position requires compassionate communication, flexibility in schedule, and the ability to work with diverse seniors.

Qualifications

• Bilingual in Korean/Vietnamese/Spanish preferred.
• Knowledge of the principles involved in relating to frail elderly persons.
• Strong communication with participants, families, and staff.
• Ability to follow directions, verbal and written instructions, and adapt schedule.
• Ability to follow through with instructions from supervisor, RN, or medical director.
• Ability to utilize safety measures when providing care.
• Confidentiality of participant information and professional conduct.
• Attend staff meetings and participate in care planning.

Responsibilities

• Participate as a member of the interdisciplinary team in ongoing participant assessment and monitoring.
• Provide personal care assistance to maintain independence in activities of daily living.
• Maintain participant safety and report changes in condition to staff.
• Escort participants to appointments as necessary and document services provided.
• Adhere to infection control, PPE usage, and fire safety standards.
• Maintain written records and timely reports of care activities.
