PACE by IIH seeks compassionate caregivers to join our interdisciplinary team and provide person-centered care for seniors at home in California. You will participate in ongoing assessments, monitor progress, and support daily living activities to promote independence and safety.

Responsibilities include helping with bathing, grooming, meals, and mobility while adhering to care plans, documenting changes, and coordinating with clinicians. Prior experience with frail elderly clients is preferred.

Qualifikationen

• Experience supporting seniors in daily living activities.
• Ability to maintain confidentiality and follow care plans.
• Ability to work with interdisciplinary team.

Aufgaben

• Participate as a member of the interdisciplinary team in ongoing participant assessment and monitoring.
• Assist with activities of daily living and personal care tasks.
• Escort participants to appointments and maintain documentation.
• Maintain infection control and safety standards.
• Support with meal service, recreation, and housekeeping as needed.
