The Pennant Group, Inc. is seeking a Nurse Assistant to join our home health team. The role supports a registered nurse and delivers essential personal care, nutritional support, and monitoring to patients in their homes.

Responsibilities include documenting observations, assisting with activities of daily living, and ensuring patient safety and comfort. A one-year hospital or nursing home experience is preferred and CPR certification may be required.

Competenze

• One-year experience in hospital or nursing home setting.
• Completed recognized nurse assistant training program desired.
• At least 18 years of age and is a high school graduate or equivalent.
• Ability to read and follow written instructions and document care given.
• Displays initiative and able to work with minimal direct supervision.
• Empathy for the needs of the ill, frail, and impaired.
• Licensed driver with automobile that is insured in accordance with the Organization requirements and is in good working order.
• Possess and maintain valid CPR certification, if required.

Mansioni

• Provide personal care including baths, back rubs, oral hygiene, shampoos, changing bed linen, dressing and undressing.
• Plan and prepare nutritious meals and assist in feeding when necessary.
• Take and record oral, rectal and axillary temperatures, pulse, respiration and blood pressure when ordered.
• Assist in ambulation and exercise according to the plan of care.
• Perform range of motion and other simple procedures as an extensional therapy service as ordered.
• Assist patient in the self-administration of medication.
• Do patient’s laundry, as appropriate.
• Meeting safety needs of patients and using equipment safely and properly.
• Report on patient’s condition and significant changes to the assigned supervisory nurse.
• Adhering to the Organization's documentation and care procedures and standards of personal and professional conduct.
