Job summary

The Pennant Group is seeking a Certified Nursing Assistant (CNA) to join its home health team. Under the supervision of a registered nurse or therapist, you will provide essential personal care and observe patients to promote safety and quality of care.

You will document observations, report changes to the supervising nurse, participate in the Interdisciplinary Group, and complete the required competency and training activities to support ongoing patient care.

Qualifications

• Must have completed a state-approved training and competency evaluation program.
• Must be at least 18 years old.
• Ability to read and follow written instructions and document care.
• Self-directed with ability to work with little supervision.
• Demonstrates empathy for the needs of the ill and impaired.
• Possess current CPR certification.
• Must have valid driver’s license and access to transportation for agency-related travel.
• Home health aide will not perform nursing duties.

Responsibilities

• Provide personal care (baths, oral hygiene, changing bed linens, dressing) and assist with toileting.
• Plan and prepare nutritious meals; assist in feeding if necessary.
• Take and record vital signs as ordered (temperature, pulse, respiration, blood pressure).
• Assist in ambulation and exercise as per plan of care.
• Attend interdisciplinary group meetings and report changes to supervising nurse.
• Complete 12 clock hours of in-service training annually.
