Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse to provide psychiatric evaluation and therapy to homebound patients. You will coordinate care with an interdisciplinary team, patients, families, and referring agencies while maintaining clinical records and ensuring medical necessity.

The role includes home visits, development of care plans, adherence to HIPAA and regulatory standards, and collaboration with physicians.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Must have 1–2 years of nursing experience (within the last 3 years preferred) in a relevant psychiatric setting.
• Current state license as a Registered Nurse.

Responsibilities

• Conduct comprehensive in-home assessments to identify physical, psychiatric, psychosocial, and environmental needs.
• Develop, implement, and regularly update individualized plans of care.
• Provide skilled nursing interventions related to behavioral health as per scope of practice.
• Coordinate care with physicians, team members, patients and families.
• Maintain accurate medication profiles and required documentation.
