Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse to provide psychiatric evaluation and therapy to homebound patients. You will coordinate care with the interdisciplinary team, patient families, and referring agencies, while maintaining accurate documentation and ensuring medical necessity for home health status.

Responsibilities include in-home assessments, individualized care planning, and supervising/Home Health Aide staff as applicable, with a focus on evidence-based

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Current state RN license required.
• 1-2 years nursing experience in psychiatric/behavioral health settings preferred.
• Valid driver's license and reliable transportation.
• CPR and Hepatitis consent/declination documentation.

Responsibilities

• Conduct in-home psychiatric assessments and determine care needs.
• Develop and update individualized care plans for behavioral health patients.
• Provide skilled nursing interventions within scope of practice.
• Coordinate care with physicians, families, and referral agencies.
• Ensure accurate medication profiles and timely admissions/recertifications/discharges.
