Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse to provide psychiatric evaluation and therapy to homebound patients. You will coordinate care with the interdisciplinary team, patient/family, and referring agency, while supervising Home Health Aides as applicable.

The role requires an RN with 1–2 years in psychiatric settings, active state license, CPR/Hepatitis documentation, and reliable transportation for in-home visits.

Qualifications

• Graduate of an accredited nursing program (diploma/associate/bachelor).
• 1–2 years nursing experience, preferably in psychiatric settings.
• RN license current; CPR and Hepatitis documentation.

Responsibilities

• Perform in-home assessments to identify physical, psychiatric, psychosocial needs.
• Develop and update individualized care plans for quality and continuity.
• Provide behavioral health nursing interventions within scope of practice.
• Coordinate care with physicians, interdisciplinary team, and family.
• Ensure accurate medication profiles and timely admissions/discharges.
• Document per regulatory standards and OASIS requirements.
