Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse to provide psychiatric evaluations and therapy to homebound patients. You will coordinate care with the interdisciplinary team, patient/family, and referring agency, while ensuring documentation supports medical necessity and homebound status.

The role requires strong assessment skills, plan-of-care development, and the ability to work with diverse teams.

Qualifications

• 1–2 years of nursing experience in acute or psychiatric settings preferred.
• Must hold a current RN license and a valid Driver's License.
• CPR certification and Hepatitis consent/declination on file.

Responsibilities

• Conduct in-home assessments to identify physical, psychiatric, psychosocial, and environmental needs.
• Develop and update plans of care to ensure quality and continuity of services.
• Provide behavioral health nursing interventions within scope of practice.
• Coordinate care with physicians, team members, patients, and families.
• Maintain accurate medication profiles and complete admissions and discharges per standards.
