Job summary

Angels Care Home Health is seeking an experienced Behavioral Health Registered Nurse to provide psychiatric evaluation and therapy to homebound patients. You will coordinate care with the interdisciplinary team, patient and family, and referring agency, while leading clinical activities and ensuring accurate documentation.

Responsibilities include in-home assessments, care plan development, nursing interventions for behavioral health, and supervision of aides.

Qualifications

• RN licensure in good standing.
• CPR certification required.
• Driver's license and reliable transportation.
• EMR proficiency preferred.
• Strong communication and collaboration skills.

Responsibilities

• Conduct in-home psychiatric and medical assessments to identify needs.
• Develop and update individualized plans of care for behavioral health.
• Provide skilled nursing interventions within scope of practice.
• Coordinate care with physicians, families, and referral agencies.
• Maintain accurate medication profiles and regulatory documentation.
• Complete admissions, resets, and discharges per standards.
• Supervise Home Health Aides and participate in QA activities.
• Ensure timely charting and compliance with timelines.
