Job summary

Angels Care Home Health seeks a Behavioral Health Registered Nurse to provide psychiatric evaluation and therapy to homebound patients. You will coordinate care with the interdisciplinary team, patients, families, and referring agencies, and manage admissions, discharges, and documentation with strict regulatory standards.

The role requires an RN license, recent psychiatric nursing experience, and the ability to work in home health with reliable transportation.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• 1–2 years of nursing experience in an acute psychiatric setting preferred.
• Current state RN license.
• Valid driver’s license.
• Proof of CPR and Hepatitis consent/declination.

Responsibilities

• Conduct in-home assessments identifying physical, psychiatric, psychosocial, and environmental needs.
• Develop, implement, and update individualized plans of care.
• Provide behavioral health nursing interventions within scope of practice.
• Coordinate care with physicians, interdisciplinary team, patients and families.
• Ensure medication profiles are accurate and current.
• Complete admissions, resumptions of care, recertifications and discharges.
• Maintain EMR data entry within required timeframes.
• Participate in QA activities and clinical audits.
• Meet productivity standards and on-call rotation including weekends.
