Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse to provide psychiatric evaluation and therapy to homebound patients in the assigned area. You will coordinate care with an interdisciplinary team, patients, families, and referral agencies, and assume responsibility for care coordination.

You will conduct in-home assessments, develop and update plans of care, perform nursing interventions, and maintain accurate documentation in the EMR while meeting regulatory standards.

Qualifications

• Graduate from an accredited nursing program (Diploma/ADN/BSN).
• 1–2 years of nursing experience in psychiatric settings preferred.
• Current state RN license.
• Current state Driver’s License.
• Proof of CPR and hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Conduct in-home assessments to identify physical, psychiatric, psychosocial, and environmental needs.
• Develop and update individualized plans of care to ensure quality and continuity of services.
• Provide skilled nursing interventions related to behavioral health and medical conditions within scope.
• Coordinate care with physicians, interdisciplinary team, patients, families, and referral agencies.
• Ensure medication profiles are accurate and current at all times.
• Complete admissions, resumptions of care, recertifications, and discharges per standards.
• Demonstrate knowledge of OASIS and ensure documentation supports medical necessity and homebound status.
• Complete visit documentation accurately within required timeframes and time-sensitive forms.
