Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse to provide psychiatric evaluation and therapy to home bound patients. You will coordinate care with the interdisciplinary team, patient/family, and referring agency, assuming responsibility for care coordination.

You will conduct in-home assessments, develop individualized care plans, and perform skilled nursing interventions within scope of practice. Strong documentation and teamwork are essential.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• Must have 1-2 years of nursing experience (within the last 3 years preferred) in a psychiatric setting.
• Current state license as a Registered Nurse.
• Current state Driver’s License.
• Proof of CPR and Hepatitis consent/declination.

Responsibilities

• Conduct comprehensive in-home assessments to identify physical, psychiatric, psychosocial, and environmental needs.
• Develop, implement, and update individualized plans of care to ensure quality and continuity of services.
• Provide skilled nursing interventions related to behavioral health and applicable medical conditions.
• Coordinate care with physicians, interdisciplinary team members, patients, families, and referral agencies.
• Complete admissions, resumptions of care, recertifications, and discharges per regulatory standards.
• Maintain proficiency in EMR systems and ensure accurate documentation.
