Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse (BHRN) to provide psychiatric evaluation and therapy to homebound patients. You will coordinate care with the interdisciplinary team, patients, families, and referring agencies, ensuring accurate medication profiles and timely documentation.

Responsibilities include in-home assessments, care plan development, and skilled nursing interventions for behavioral health, while meeting regulatory standards and productivity

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• 1-2 years of nursing experience (within the last 3 years) in psychiatric/acute settings preferred.
• Current state license as a Registered Nurse.
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Responsibilities

• Conduct comprehensive in-home assessments identifying physical, psychiatric, psychosocial, and environmental needs.
• Develop, implement, and update individualized plans of care for quality and continuity of services.
• Provide skilled nursing interventions related to behavioral health within scope of practice.
• Coordinate care with physicians, interdisciplinary team, patients, families, and referral agencies.
• Ensure patient medication profiles are accurate and current at all times.
• Complete admissions, resumptions of care, recertifications, and discharges per regulatory standards.
