Angels Care Home Health is hiring a Behavioral Health Registered Nurse to provide psychiatric evaluation and therapy to homebound patients. The role coordinates care with the interdisciplinary team, families, and referring agencies while overseeing care plans and documentation.

The candidate will perform comprehensive in-home assessments, implement plans of care, and supervise aides as applicable in a home health setting.

Qualifikationen

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• 1-2 years of nursing experience within the last 3 years preferred in psychiatric settings.
• Current state RN license.
• Current state Driver’s License.
• Proof of CPR and Hepatitis consent/declination.
• Reliable transportation with valid auto liability insurance.

Aufgaben

• Conduct comprehensive in-home psychiatric assessments and care planning.
• Provide skilled nursing interventions for behavioral health patients.
• Coordinate care with physicians, families, and referral agencies.
• Ensure medication profiles are accurate and updated.
• Complete admissions, resumptions, recertifications, and discharges per standards.
• Maintain EMR documentation including SNVs and OASIS.
