Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse (BHRN) to provide psychiatric evaluation and therapy to homebound patients. You will coordinate care with the interdisciplinary team and referrers while maintaining care plans and documentation.

The role requires an active RN license, 1–2 years in psychiatric settings preferred, and strong EMR/OASIS proficiency. Travel to patient homes is required as part of patient care, with full benefits and growth opportunities.

Qualifications

• Graduate of an accredited diploma, Associate or Baccalaureate School of Nursing.
• 1–2 years nursing experience in psychiatric settings preferred.
• Current RN license & valid driver's license.
• CPR certification and Hepatitis consent/declination.
• Reliable transportation with auto insurance.

Responsibilities

• Conduct in-home psychiatric assessments to identify clinical and psychosocial needs.
• Develop and update individualized care plans for quality and continuity of services.
• Provide behavioral health nursing interventions within scope of practice.
• Coordinate care with physicians, team members, patients and families.
• Maintain accurate medication profiles and compliant documentation.
• Complete admissions, resumptions of care, and discharges per standards.
• Demonstrate proficiency with OASIS and ensure medical necessity.
• Complete visit documentation including SNVs and OASIS within required timeframes.
