Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse to provide psychiatric evaluation and therapy for home-bound patients. The RN coordinates care with the interdisciplinary team, patients, families, and referring agencies while maintaining accurate documentation and medication profiles.

The ideal candidate has 1–2 years of psychiatric nursing experience, an active RN license, CPR certification, and reliable transportation. A driver’s license is required for in-home visits.

Qualifications

Must be a graduate of an accredited nursing program and be eligible for RN licensure.
1–2 years of nursing experience in psychiatric settings preferred.
Current state RN license and CPR certification required. Hepatitis consent/declination as applicable.

Responsibilities

Perform in-home psychiatric assessments and develop care plans.
Provide behavioral health nursing interventions within scope of practice.
Coordinate care with interdisciplinary team, patients, families, and referral agencies.
Maintain accurate medication profiles and timely documentation of visits and OASIS.
Complete admissions, resumptions of care, recertifications and discharges.
Participate in QA activities and ensure regulatory compliance.
