Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse to provide psychiatric evaluation and therapy to homebound patients, coordinating with the interdisciplinary team and families. This role requires an RN license, 1–2 years of psychiatric nursing experience, and a valid driver’s license.

Qualifications

• Must have nursing degree from an accredited program (Diploma/ADN/Bachelor).
• 1–2 years of nursing experience, preferably in psychiatric settings.
• Current RN license in the state of practice.
• Valid driver’s license and reliable transportation.
• CPR certification and hepatitis consent/declination.
• Reliable transportation with auto liability insurance.

Responsibilities

• Conduct in-home assessments of physical, psychiatric, psychosocial and environmental needs.
• Develop and update individualized care plans for quality and continuity of services.
• Provide behavioral health nursing interventions within scope of practice.
• Coordinate care with physicians, interdisciplinary team, patients and families.
• Ensure accurate medication profiles and timely admissions, recertifications and discharges.
• Maintain EMR documentation and ensure timely visit entries.
• Participate in QA activities, case conferences and on-call rotation.
