Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse (BHRN) to provide psychiatric evaluation and therapy to home-bound patients. The role coordinates care with the interdisciplinary team, patient/family, and referring agency, while maintaining medication profiles and completing admissions, resumption of care, recertifications, and discharges in line with regulatory standards.

Qualifications include RN license and 1–2 years of psychiatric/nursing experience.

Qualifications

• Graduate of an accredited nursing program (diploma/associate/baccalaureate).
• 1-2 years of nursing experience in psychiatric/acute settings preferred.
• Current RN license and valid driver's license.

Responsibilities

• Conduct comprehensive in-home assessments to identify physical, psychiatric, psychosocial, and environmental needs.
• Develop, implement, and regularly update individualized plans of care to ensure quality and continuity of services.
• Provide skilled nursing interventions related to behavioral health and applicable medical conditions within scope of practice.
• Coordinate care with physicians, interdisciplinary team members, patients, families, and referral agencies.
• Ensure patient medication profiles are accurate and current at all times.
• Complete admissions, resumptions of care, recertifications, and discharges in accordance with regulatory standards.
• Demonstrate knowledge of OASIS and ensure documentation supports medical necessity and homebound status.
• Monitor assigned cases to ensure compliance with payer, federal, and state requirements.
• Supervise Home Health Aides as applicable and participate in QA activities.
