Job summary

Qualifications

• Graduate nursing degree or diploma (RN).
• 1–2 years nursing experience in psychiatric/behavioral health settings preferred.

Responsibilities

• Conduct in-home psychiatric assessments and psychosocial evaluations.
• Develop and update individualized care plans to ensure quality and continuity of services.
• Provide skilled nursing interventions for behavioral health and related medical needs.
• Coordinate care with physicians, families, and referral agencies.
• Maintain accurate medication profiles and ensure proper documentation including admissions and discharges.
