Job summary

Angels Care Home Health is seeking a Behavioral Health Registered Nurse (BHRN) to provide psychiatric evaluation and therapy to homebound patients. You will coordinate care with the interdisciplinary team, families, and referral agencies, while maintaining accurate documentation and regulatory compliance.

The ideal candidate has 1–2 years of nursing experience in psychiatric settings, an active RN license, and reliable transportation.

Qualifications

• Graduate of an accredited nursing program (Diploma/ADN/BSN).
• 1–2 years of nursing experience in acute or psychiatric settings preferred.
• Current state RN license in good standing.
• Current state Driver’s License.

Responsibilities

• Conduct in-home assessments to identify physical, psychiatric, psychosocial, and environmental needs.
• Develop and update individualized plans of care for home health patients.
• Provide skilled nursing interventions for behavioral health conditions.
• Coordinate care with physicians, families, and referral agencies.
• Complete admissions, re-certifications, and discharges per regulatory standards.
• Maintain accurate OASIS documentation and medical necessity.
