Job summary

Integrated Services for Behavioral Health in Fairfield County, OH is seeking a Care Coordinator to support children and families with behavioral health needs. The role focuses on service linkage, coordinating care in home, school, and community settings, and ensuring plans meet safety and treatment goals.

Qualifications

• Experience providing services to children and families in behavioral health or related fields.
• Knowledge of local community resources and care coordination with child and family teams.
• Valid Driver's License.
• Ability to operate in an Internet-based, automated office environment.
• Hi-Fidelity Wraparound certification preferred (provided at time of employment).
• Two+ years in a coordinated supportive services or care coordination role preferred.

Responsibilities

• Identify care coordination needs with families to align with service delivery standards.
• Define cultural factors influencing strengths and family dynamics to support care planning.
• Identify strengths of children, youth, and families for engagement in care coordination.
• Coordinate family-based services in home, school, and community.
• Ensure services on care plans are appropriate and safe.
• Engage families and build relationships to support integrated care plans.
• Develop partnerships with community resources to meet diverse needs.
• Maintain documentation and participate in program evaluation and planning.
