Job summary

Integrated Services for Behavioral Health (ISBH) is seeking a Care Coordinator in Fairfield County, OH. The role supports youth and families at home, school, and in the community, coordinating services and ensuring voice and choice in care planning.

The position pays $20.19–$25.03 per hour, with eligibility for a $3,000 hiring bonus. Travel within the service area is required, and experience with behavioral health and Medicaid systems is preferred.

Qualifications

• Experience serving children and families in behavioral health or related fields.
• Three years with a HS diploma or two years with an associate/bachelor's, or one year with a master's.
• Knowledge of Hi-Fidelity Wraparound preferred; certification provided at start.
• Two years in a care coordination role emphasizing organization and attention to detail.
• Experience with autism/developmental disabilities preferred.

Responsibilities

• Lead and organize a team for youth and family goals and action steps.
• Coordinate care in home, school, or community settings.
• Identify services needed and ensure the youth/family voice in care decisions.
• Maintain required documentation and access OhioRISE benefits.
• Attend trainings and maintain certifications as required.
• Utilize HIPAA compliant practices and secure data in EHR systems.
• Travel within the service area and meet deadlines in a high-volume environment.
