• Preference for Hi-Fidelity Wraparound certification and cross-systems care coordination.

Responsibilities

• Identify care coordination needs with families to ensure best outcomes.
• Coordinate family-based services in home, school and community settings.
• Maintain documentation and participate in program planning and training.
• Engage with child and family teams to support care plans and outcomes.
