Integrated Services for Behavioral Health is seeking a Care Coordinator in Fairfield County, OH to connect families with needed services, coordinate care across home, school, and community, and work with Child and Family Teams to ensure voice and choice in care planning.

The role emphasizes cultural humility, collaboration, and building partnerships with community resources. A $3,000 hiring bonus is offered to new team members joining ISBH.

Formación

• Experience providing services to children and families in behavioral health or related public sector human services.
• Must have a valid driver's license and ability to operate in an internet-based, automated office environment.
• Ability to coordinate care across home, school, and community with families and Child and Family Teams.

Responsabilidades

• Identify care coordination needs with families to meet program outcomes.
• Define cultural factors influencing engagement and family interaction for successful care planning.
• Coordinate family-based services in home, school, and community.
• Ensure services on plans are appropriate, least restrictive, and meet safety and treatment needs.
• Engage families to support integration of team members and care plans.
• Develop partnerships with community resources to meet diverse needs of youth and families.
• Maintain documentation, participate in program evaluation, and attend planning meetings.
