Job summary

Alignment Health seeks an Advanced Practice Provider (NP/PA) to manage complex patients in a home-based, collaborative care model. The role includes in-home assessments, some outpatient work, and close coordination with physicians, social workers, and care managers.

The position emphasizes end-of-life discussions, patient education, and data-driven care using Epic EMR and a proprietary decision support tool. 3.5 days on-site/community visits and 1.5 days remote work per week are available.

Qualifications

• Active state license as a Nurse Practitioner or Physician Assistant.
• Board Certification in NP or PA.
• Medicare Part D Prescriber credential.
• Valid driver license and automobile insurance.

Responsibilities

• Conduct in-home assessments (max 5 per day) and some outpatient clinical work.
• Collaborate with physicians, APNs, social workers, case managers, and other team members.
• Communicate findings to PCP to close care gaps.
• Provide patient/family education and follow-up planning.
• Adhere to HIPAA and protect PHI.
