Job summary

Alignment Health is seeking an Advanced Practice Clinician for the Care Anywhere program to deliver in-home assessments, education, and care coordination with a multidisciplinary team. Ideal candidates hold an NP or PA Master’s degree, active licensure, board certification, and current BLS.

Qualifications

• Master’s degree from an accredited NP or PA program.
• Active NP/PA license; RN license required upon hire.
• Board certification in NP or PA; valid BLS; NPI/DEA numbers.

Responsibilities

• Conduct in-home assessments and medication review.
• Provide patient education and align care with patient needs.
• Identify diagnoses to support care management and medical management.
• Coordinate with Regional Medical Officer and team to develop care plans.
• Collaborate with PCPs, specialists, and ancillary providers; lead a clinical team.
