Job summary

Monogram Health is seeking Advanced Practice Providers (NP/PA) to deliver in-home and telehealth care for patients with multiple chronic conditions. You will join an integrated team, build long-term relationships, and support a value-based care model.

Role includes 3-4 in-home visits weekly, no on-call or weekend requirements, and competitive base salary with bonuses, mileage reimbursement, and CEU/CME support.

Qualifications

• Active and unrestricted RN and NP license, or PA license.
• Board certification (NP: ANCC/AANP | PA: NCCPA).
• Current and unrestricted DEA certificate, or eligible to obtain.
• Minimum of two years of direct patient care experience.
• Field-based role requiring travel to patient homes and reliable transportation.
• BLS certification.
• Knowledge of HIPAA/OSHA regulations.
• Experience with EHR systems (Athena, Salesforce, Epic).
• Background in managed care or population health preferred.

Responsibilities

• Provide evidence-based, patient-focused care directly in the patient’s home.
• Perform comprehensive assessments and wellness visits.
• Educate patients and caregivers on disease management and medications.
• Participate in recurring virtual integrated care team meetings.
• Engage in quality assurance and documentation to ensure compliance.
• Practice in various settings (SNFs, ALFs, hospitals) as applicable.
• Perform point-of-care testing, prescribe meds, order tests, and collaborate with the Monogram physician.
• Coordinate care with PCPs, specialists, and ancillary services.
• Complete documentation accurately and timely.
