Job summary

Alignment Health is seeking an Advanced Practice Clinician to support the Care Anywhere program by delivering in-home medical and social support to our patients. The role focuses on preventing hospitalizations and managing chronic conditions with timely clinical interventions.

You will conduct in-home assessments, review medications, educate patients, and coordinate with PCPs and specialists to develop comprehensive care plans. A master’s NP/PA degree and current licensure are required.

Qualifications

• Master’s degree from an accredited NP or PA program is required.
• Active NP or PA license is required (RN with furnishing number permitted for NP).
• Ability to communicate positively and effectively with others; strong problem solving.
• Experience in geriatric or home-based care preferred; EMR experience preferred.

Responsibilities

• Conduct in-home assessments and health screenings for Alignment members.
• Review medications and provide patient education aligned with care plans.
• Identify diagnoses to be addressed in care management and active medical management.
• Collaborate with Regional Medical Officer and team to develop care plans.
• Coordinate care with PCPs, specialists, and ancillary providers.
• Lead clinical team including nurses, health coaches, social workers and coordinators.
