Job summary

Alignment Health, LLC is seeking an Advanced Practice Provider (NP/PA) to manage complex, high-risk seniors in a home-based care model. You will collaborate with physicians, case managers, and social workers to deliver coordinated care and ensure HIPAA/compliance standards.

Qualifications

• Active state NP/PA license and board certification required.
• Prior clinical experience, preferably in geriatrics or home health.
• Experience with EPIC EMR and patient monitoring systems.

Responsibilities

• Conduct in-home assessments and limited outpatient work 3.5 days per week.
• Perform comprehensive history, exam, med review and screenings.
• Coordinate with PCP, social workers and care team; discuss end-of-life if needed.
