Job summary

Monogram Health is seeking Advanced Practice Providers (NPs or PAs) to deliver high-quality in‑home care to patients with multiple chronic conditions. This role operates within an integrated multidisciplinary team and supports a value‑based, patient‑centered care model both in home visits and via telehealth.

Qualifications

• Active/unrestricted RN/NP license or PA license required.
• Board certification required (NP or PA).
• Current unrestricted DEA certificate or eligibility to obtain.

Responsibilities

• Provide patient-centered care in the patient’s home to reduce hospitalizations.
• Perform comprehensive assessments and follow-up visits including wellness exams.
• Educate patients and caregivers on disease management and preventive care.
• Participate in virtual integrated care team meetings.
• Coordinate care with PCPs and specialists and document promptly.
