Alignment Health is seeking an Advanced Practice Clinician for the Care Anywhere program to provide home-based medical and social support to our patients. This role focuses on delivering high-quality care in the comfort of patients' homes while coordinating with the broader care team.

You will perform in-home assessments, review medications, educate patients, and help design care plans in coordination with regional leaders and PCPs.

Qualifikationen

• Master’s degree from an accredited NP or PA program.
• Active Nurse Practitioner or Physician Assistant license.
• One year of clinical or home care experience preferred.
• EMR experience preferred.
• Experience with geriatric patients preferred.

Aufgaben

• Conduct in-home assessments on Alignment members and review medications.
• Provide patient education by matching care desired with best care given.
• Identify diagnoses to be assessed in care management and active medical management.
• Collaborate with Regional Medical Officer and team to develop appropriate care plans.
• Coordinate care with PCPs, specialists, and auxiliary providers.
• Lead a broader clinical team including nurses, health coaches, social workers and care coordinators.
