Job summary

Alignment Health seeks an Advanced Practice Clinician to join the Care Anywhere program, delivering in-home medical and social support to enrolled patients. This role emphasizes compassionate care, timely interventions and patient-centered planning right in the patients' homes.

The APC will perform home visits, review medications, conduct health screenings, educate patients, and coordinate with primary care and specialists while leading a clinical team when needed.

Qualifications

• Master’s degree from an accredited NP/PA program.
• Active NP or PA license and furnishing number.
• Active RN license with furnishing number (must, upon hire).
• NP/PA board certification (NPI number, DEA, valid BLS).
• Valid driver’s license and current automobile insurance.

Responsibilities

• Conduct in-home assessments for Alignment members, medication review and health screenings.
• Provide patient education by matching care desires with appropriate care.
• Identify diagnoses to be assessed in care management and active medical management.
• Coordinate care with PCPs, specialists and ancillary providers; discuss and develop care plans.
• Lead broader clinical team which may include nurses, health coaches, social workers and care coordinators.
