Job summary

Alignment Health is seeking an Advanced Practice Clinician for the Care Anywhere (CAW) program to deliver in-home medical and social support. The clinician will assess patients, review medications, and conduct health screenings to prevent hospitalizations and disease progression.

Requirements include a Master’s degree in an NP or PA program, active NP/PA license with board certification, and strong communication and problem-solving skills.

Qualifications

• Active NP or PA license with board certification.
• Excellent communication, problem-solving and time-management skills.

Responsibilities

• Conduct in-home assessments, medication review and health screenings.
• Provide patient education aligning care needs with available options.
• Identify diagnoses to guide care management and medical decisions.
• Coordinate care with PCPs, specialists and ancillary providers.
• Lead broader clinical team including nurses, health coaches and care coordinators.
