Job summary

Community Guidance Center in Indiana is seeking a full-time Supports Coordinator to locate, coordinate, and monitor services for individuals on their caseload. The role requires meeting consumers in their homes, day programs, and community settings, with a flexible schedule and strong emotional intelligence to meet needs.

Responsibilities include drafting and maintaining ISP plans, coordinating with families and providers, and ensuring health and safety through effective monitoring.

Qualifications

• Bachelors degree including 12 credits in related social sciences.
• Valid Pennsylvania driver’s license.
• Act 33 & 34 Clearances and FBI Clearance and Act 31 Mandated Reporter Training.
• Intellectual Disabilities experience preferred.
• Must demonstrate emotional intelligence in dealing with conflict between staff and/or consumers.
• Must have and maintain a valid driver’s license and vehicle for appointments outside the office.

Responsibilities

• Locate, coordinate, and monitor services and supports for individuals.
• Write and maintain an Individual Support Plan (ISP) for each consumer.
• Coordinate ISPs with participants, families, ISP team, and providers.
• Schedule and monitor services in home, day program, or community settings.
• Complete required paperwork within timelines and adhere to policies.
• Attend mandatory trainings and monthly meetings; participate in supervision.
• Participate in a 24-hour on-call system for emergencies/crisis.
• Follow ODP regulations and Sanctuary commitments in daily work.
