Job summary

YAI is seeking a dedicated Support Coordinator in New Jersey to assess client needs, secure Medicaid and DDD-funded services, and develop person-centered ISPs. You will coordinate service delivery, monitor care, and document progress, including on-call support as needed.

The role includes in-home visits, collaboration with families and providers, and ensuring services stay within budget. A Bachelor’s degree and experience with the adult I/DD population are preferred.

Qualifications

• Bachelor’s degree required.
• At least 1 year of experience with adult I/DD population.
• Ability to be on-call beyond regular hours.

Responsibilities

• Assess client needs and facilitate access to services.
• Determine eligibility and secure Medicaid and DDD-funded services.
• Develop and monitor Individualized Service Plans (ISPs).
• Coordinate ISP implementation and connect to service providers.
• Monitor services within budget and update ISPs.
• Conduct in-home visits and document findings.
