Responsibilities

• Liaison with multiple agencies and stakeholders to coordinate services for individuals.
• Documentation and record keeping for regulatory and reimbursement needs.
• Assess eligibility and plan services based on needs and supports.
• Regularly monitor and review progress, updating ISP and outcomes.
• Refer to residential, employment, and community supports as needed.
• Develop annual ISP with a team and ensure timely reviews.
