Qualifications

Responsibilities

Assess needs to determine eligibility for intellectual disability services and develop individual service plans.
Link individuals to services and supports specified in the ISP.
Arrange transportation for individuals or family members to access services.
Coordinate services and benefits with other agencies and providers.
Provide crisis intervention and advocacy for individuals and families.
Maintain documentation in the Electronic Health Record and other platforms for reimbursement and monitoring of services.
Attend in-service trainings to enhance case management services.
