• Bachelor's degree in behavioral health or related field is required.
• Experience with case management for individuals with developmental disabilities.

• Strong communication and collaboration with families and providers.

Responsibilities

• Assess needs to determine eligibility for intellectual disability services and create an ISP.
• Link individuals to services and supports per their ISP and crisis services if needed.
• Coordinate transportation and access to services for individuals and families.
• Maintain documentation in electronic health records for reimbursement and reporting.
• Participate in treatment teams and advocate for appropriate interventions.
• Monitor service delivery and community integration goals improvements.
